August 8: Sports Physical Day at LMHS

On Tuesday, August 8, 2023 the Northwest CHC will be offering Sports Physicals at no cost to parents for middle and high school students. Physicals will take place in the LMHS Gymnasium. Please arrive within the time slot assigned for first initial of your last name:

• 8 am – 10 am for last names that begin with A – F
• 10 am – 11:45 am for last names that begin with G – L
• 12:30 pm – 2 pm for last names that begin with M – R
• 2 pm – 4 pm  for last names that begin with S – Z

The front office does not take copies of physicals, but parents or students should upload both pages of their physical to their DragonFly account (Instructions Below).

Any student who wants a sports physical must either have their parent/guardian present to check them in, or have a permission slip signed by their parent/guardian and present it at check in. Physical Permission Form (required for physical) & Vaccines Permission Form (optional):
