The Lincoln County Health Department will have a flu shot clinic at LMHS for staff, and for students who have the authorization form completed by a parent/guardian. If you would like your child to be given the flu shot by the Lincoln County Health Department, please complete the attached form and have it returned to the LMHS office as soon as possible before the clinic begins.  The forms are also available to grab from the LMHS office (on the counter).
